Supports include Home Care, allied health services: physiotherapy, occupational therapy, podiatry and social support groups.

About the program
Macedon Ranges Health delivers the Home and Community Care Program for Younger People (HACC-PYP) in the Macedon Ranges Shire. This program is funded by the Victorian Government and is designed to support eligible individuals to stay independent at home and in the community by providing them access to a range of affordable health care and support services.If eligible for HACC-PYP the Victorian Government will cover the cost of a range of services, ensuring you can access the support you need. You will be required to make a small co-contribution to the cost of your services. This fee will vary depending on your income. At assessment we will chat with you to determine your income and factors that may affect your ability to pay fees. Fees may be waived in circumstances of financial hardship.
Who is Eligible for HACC PYP?
You may qualify for HACC-PYP services if:
• You are under 65 years old (or under 50 if Aboriginal or Torres Strait Islander)
• You have a chronic illness, disability, injury, or mental health condition affecting your independence
• You do not have NDIS funding but require support to continue living at home.
Our Services
Macedon Range Health offer a range of affordable health care and support services to people eligible for HACC PYP funding, including:
• Home Care Services – Assistance with daily living tasks to help you remain at home safely such as personal care, domestic assistance, meals, home maintenance.
• Allied Health Support:
- Physiotherapy – Improve mobility, strength, and physical function including exercise classes
- Occupational Therapy – Enhance daily living skills and home safety
- Podiatry – Foot care for better mobility and health
- Dietetics – to assist with managing dietary needs related to your medical condition/health
• Nursing - including continence nursing
• Social Support Individual or Groups – Stay socially connected and engaged through community activities
